Abstract
Metformin is one of the most common medications used worldwide to treat type 2 diabetes (T2D). However, about one-third of patients respond poorly to metformin. The role of the microbiome and its metabolic signature on the pharmacogenomics of metformin has become an area of increasing interest. This proposal aims to investigate the pharmacogenomics of poor versus good metabolizers of metformin users, the associated metabolic signature of microbiota's alpha diversity, and their potential impact on drug response. Our preliminary findings have demonstrated a substantial inter-individual variation in the metabolic response to metformin, which appears to be influenced, in part, by the diversity of gut microbiota. Notably, specific patient subgroups exhibited unique microbial metabolic profiles and varied responses to metformin, highlighting the importance of considering both genetic and microbiome composition. These findings emphasize the potential of pharmacometabogenomics in personalized medicine, particularly for optimizing metformin therapy. Through a comprehensive approach, we will analyze genetic and microbiome data obtained from a cohort of T2D patients undergoing metformin treatment. Our research strategy involves two integral arms: the retrospective arm will explore the association between genotypes, metabolic pathways, clinical traits, and metformin response in a cohort of 400 T2D patients from Qatar Biobank.
